Boarding Medical Consent Form

At Southlake Crossing Animal Clinic, we take great pride in the way we care for our boarding guest while they are here visiting. We will not, at any time, allow a pet in our care to be uncomfortable or suffer, and go without medical attention. If your pet becomes ill or injured while in our care, we make every attempt to contact you first and/or your emergency contact. It is extremely important that you provide accurate telephone numbers where you can be reached. In the case that you cannot be reached, before you leave on your trip, please discuss with your emergency contact, your wishes for the care of your pet. If we need to contact you for any medical reason concerning your pet, your decisions will be in writing and another staff member will speak to you to witness your decisions. In the event we are unable to reach you or your emergency contact, we will provide the basic services necessary to stabilize your pet to make him/her as comfortable as possible until your or your contact person can be reached.

Transfer my pet to The Animal Emergency Hospital of North Texas for after hours care.
Transfer my pet to Southlake Crossing Animal Clinic for day treatment.
Consent: I agree to the above terms for boarding. I accept all financial responsibility for any and all medical care provided by Southlake Crossing Animal Clinic/The Animal Emergency Hospital of North Texas
