Please feed my pet:

If your pet is not current on the vaccinations against Rabies, DHPP, Leptospirosis and bordetella for dogs, and FVRCP and Rabies for cats they will be vaccinated at the owners expense.  All pets will be examined for fleas and ticks, if found, the animal will be treated at the owners expense.  If a pet breaks out with gastrointestinal problems (vomiting or diarrhea), they will be treated at the owners expense. Additionally, Southlake Crossing Animal Clinic is not responsible for lost or damaged items left with my pet during their stay.  In the event of an emergency, every reasonable attempt will be made to contact you or your emergency contact listed above.  If we are unable to reach you, or if treatment is necessary immediately, all related expenses will be the responsibility of the owners.
